The undersigned hereby acknowledges that FloorWorks has determined that proceeding with the installation of the materials and conditions described above is not in conformance with industry standards and therefore will void all warranties. FloorWorks recommends that the installation does not continue until such time the aforementioned concerns are completely corrected.
By signing below, the builder is directing FloorWorks to proceed with the installation and builder accepts full financial responsibility for all future claims, including warranty, related to the products and installation as a result of proceeding with installation.
